In respect of Patients/ dependents( in case of Minors and /or with special needs)

Name, Personal email ID, Phone/Moile Number, Gender, Age, Date of birth, Health related history (like Medical, Surgical, hospitalization history, allegoric to medicine(s) etc.), Health related concern and observations (like fever, fatigue, weakness, weight loss, allergies, Blood pressure, persistent medications, Tobacco/ alcohol or other drug intake, Heart disease), Some of these data could be optional.

Purpose of Processing

For health service consultation/ examination/dialogue/review (including video consultation, in-person consultation with specialist etc.) for you and your dependents with Heart Team India’s Doctors/ Nurses/ Paramedical and Clinical Staff, as applicable, in case of illness/health difficulties/ complication/pain.
For providing any health-related data (including reports etc.) as part of your health consultation and diagnosis/ discovery/analysis/examination.

Data Recipients

Your personal data will be accessible to Heart Team India its, doctors/nurses/paramedical staff and their associates, authorized pharmacies and authorized laboratory(s) of HTI as applicable.
In case of patients who are minors and/ or with special needs, it will be accessible to their respective guardian.
Your personal data may also be accessible to authorities/auditors/ lawyers as permitted by law.

Data Storage:

Your data will reside on our authorized cloud service providers’ servers located in India. Such data may also be stored on Heart Team India’sservers.

Security Measures:

Reasonable measures will be taken to prevent unauthorized access, unlawful processing, unauthorized or accidental loss or destruction of the collected Personal Data. However, please also remember that we cannot guarantee that the internet itself is 100% secure. Our website/apps links to other website/apps that may collect personally identifiable information about you. We are not responsible for the privacy policy or the contents of those linked website/apps. We may use also your personal information for analysis of data, usage trends and to evaluate and improve our site/App, marketing research, preventing of frauds. In our efforts to continually improve our product and service offerings, we collect and analyse demographic and profile data about our users’ activity on our website/apps.
